By signing this Volunteer Application Form, you affirm that all the information provided is accurate and that you have fully disclosed all relevant information. If you are the parent or guardian of a minor applying to serve through this application, you are consenting to this individual volunteering at the church.


SUPPORTING DOCUMENTS CHECKLIST:

The checklists below will tell you what additional documents are required to serve in the various sensitive roles in our church. Please find the role you are applying for to see what additional documents we need from you.

After you start volunteering, you will be required to resubmit all documents every five years if you continue serving in the same or a similar role. This helps us ensure that our church remains a safe environment for everyone.
Criminal Record Check (If over 18 years of age)
You get this from the police in the municipality where you currently live. If there is a cost, we will reimburse you. Submit your receipt through the Tithes and Offering box.

Signed Privacy Policy Acknowledgment
You can find a copy of our Policy for Protecting Privacy on the Volunteer page. Once you have read the policy please sign below.

Vulnerable Sector Check (If over 18 years of age)
You get this from the police in the municipality where you currently live. You will need a letter requesting a VSC from us, please request one through the contact form at the bottom of this page. Please check the ID requirements of your local police department. If there is a cost, we will reimburse you. Submit your receipt through the Tithes and Offering box. And hand in your VSC once you have received it to a member of the Servant Team or in the Tithes and Offering box.
